The Case Manager is a vital member of the Home Medical Services Team, responsible for providing comprehensive care management and remote monitoring to medically complex patients. In this hybrid clinical and care management role, you will develop and implement personalized care plans, coordinate and supervise patient care, and provide health education and preventive screenings to patients and their caregivers—primarily conducted remotely, with occasional in-person home visits as needed.

Essential Job Duties

  • Conducts thorough assessments of patients and develops care plans and goals tailored to each patient’s unique medical, social, financial, and emotional needs.
  • Guides patients through the transition between different care settings and supports their return to the community.
  • Prioritizes patient care based on clinical assessments, reports, biometrics, and physician orders, using strong critical thinking skills.
  • Ensures that documentation of care is clear, accurate, timely, and meets all regulatory standards.
  • Collaborates and communicates with all healthcare providers involved in a patient’s care for seamless coordination and consistent outcomes.
  • Regularly monitors patient progress and communicates updates and care plan changes with the clinical team.
  • Stays current on best practices and required training through ongoing education.
  • Leads interdisciplinary case discussions to troubleshoot challenges and plan next steps.
  • Performs additional duties as assigned.

Position Details:

Working Conditions: Hybrid role; primarily remote but some onsite time required

Travel: May be required to conduct home visits with patients as deemed necessary for care mgmt. related activities in accordance with standard operating procedures

Shift: Regular FT 40 hrs/week; Day Shift

Qualifications:

  • Graduate of an accredited school of nursing, BSN preferred
  • Current licensure as RN in Maryland
  • Current CPR certification
  • Valid Maryland drivers’ license and current auto insurance, working vehicle; agree to comply with Maryland law for mandatory seat belt use
  • Must live in Maryland
  • 3 years' experience in case management and knowledge of community resources
  • Experience working as part of a multi-disciplinary team as well as with people of diverse backgrounds preferred
  • A minimum of 2 years' experience working with adult patients with multiple comorbidities
  • Home care experience preferred

Salary Range: Minimum 36.31/hour - Maximum 56.28/hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility. In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.
